All the samples sent to the Laboratory should be accompanied by Waikato Radiocarbon Dating Laboratory sample submission forms. Please take time out to fill out the details on the submission forms. They will stay with the sample records in the database, and are available for future reference

Online form
The Waikato Radiocarbon Dating Laboratory's online sample submission form enables fast and easy input of data into our lab database.
Word Doc/PDF form
Alternatively, you can submit a PDF copy or Word Doc version of the sample submission form.
To print the PDF you will need to have Adobe Acrobat Reader installed.
Note:
Please contact us to get a copy of our current MPI permit and include this in the package when you send us material from outside New Zealand.
Please also include paper copies of the sample submission forms with your samples.
